Miller class IV gingival recessions are characterized by severe interproximal hard and soft tissue loss. The interdental tissue is apical to the buccal gingival margin. Root coverage procedures are commonly not indicated since complete root coverage is unpredictable and virtually impossible to achieve. Miller class IV recession type defects are subject to additional therapies (e.g. orthodontics).
